Increasing infertility problem is important modern life issue. Couple with infertility problem will do everything to achieve successful pregnancy. However, most do not have proper documentation of the treatment. Improper documentation impaired the continuation of care, affecting their precious time, also financially. This prototype aimed to document details on their infertility problem, so that advancement in the management could be offer to the couple. Besides giving information on infertility, it record couple’s profile, investigations report, and treatment history in a nicely design A5 booklet that may give positive emotional support as they sailed their anxiety-distressing infertility journey.
